Connectedly’s mission is to strengthen community support networks, reduce social isolation, and improve the well-being of older adults, especially women and their families. They are focused on Grandparent caregivers, older adults living independently, and homebound adults. Connectedly is guided by its mission and centered on its vision of enhancing equity and well-being of low-income older women of color in Philadelphia.

Through GEEP, Connectedly hopes to learn from the other members of the Learning Cohort about their efforts to strengthen innovative care models that are equitable, inclusive, and promote long-term systems change. Connectedly is built on a peer group model; they know the tremendous value of learning from each other, exchanging ideas, providing mutual support, and working collaboratively to effect change. They hope to work together with the other grantee organizations to learn about how to design effective equitable strategies, communities, and systems. Connectedly looks forward to the opportunity in the Community of Practice to share their knowledge, experiences, successes, and challenges as they relate to providing gender equity with the other members.
